    Could someone please explain what is going on with this Phormium flower spike and why. We are told this particular plant has done this regularly but that others in the area have produced the same phenomenon.

    Someon might correct me, but this looks like fasciation, which is not uncommon (especially in certain genera, such as Echium). This kind of development is not uncommon with Phormium cookianum, especially in planted specimens that might be experiencing stress.
